The operator will include in each monthly news letter a designated time for parent(s)/guardian(s) to come and have a “getting to know you meeting”. This meeting is between the parent(s)/guardian(s), teacher(s), and director. Topics discussed involve family composition, child and parent information in order to provide quality service to you and your child’s needs.
The following are forms needed for your child’s enrollment,
you can either print it or get it from our center.
Emergency Form – This must be filled out completely, including emergency contacts
and persons designated to pick up a child. Parents/Guardians are
required to notify the Center immediatelyof any changes of name,
addresses, and/or telephone numbers provided on this form for
parents/guardians or any other emergency contact person.
Child Health Report –
In accordance with Pennsylvania’s State regulations, parents must submit a completed
health appraisal. Verification of immunizations must be presented at the time of enrollment.
Parent Agreement –
Parents must sign this form agreeing to the payment policies for the services requested.
All parents are required to sign and update the parent agreement form annually.
